The Team Support Program (TSP) is an 8-bed intensive residential home with four distinct two person apartments located in Manchester. The home and programming are designed to provide trauma informed care to adults previously living in long-term care at Connecticut Valley Hospital who have Severe and Persistent Mental Illness (SPMI) and experience extreme states. Clinical supports focus on positive behavioral strategies. All supports provided are person centered and include a multi-disciplinary approach to meet the unique needs of the individual. |
